I used
Hackham Precision Engineering to make up the dogbones for me,

The dogbone is designed to have an offset of 7mm initially for trial fitting.
You mount everything up and check that the caliper is centered on the rotor.
If it isn't, the offset can be machined until it is centered.
I always advise making up a prototype dogbone from ply/MDF etc. so that you can be 100% familiar with what is required before paying $$ on the machining of the real thing
I used
API Engines/Rising Sun Imports to machine the rotors.

Terry, at API, is an ex neighbor of mine and he enlarged the centre hole and re-drilled the rotors for me to suit the 200B hubs I gave him.
It all fitted perfectly
I'm sure there are other Shops that would do this for you closer to home if you aren't "down South".
Do you need the CAD drawing of the dogbone?
If you do, I'll have to email it to you, I can't post .cad files to the Forum.
